Generic vs. branded medicines in India: what's the difference?

Why a ₹4 generic paracetamol can be as effective as a ₹40 branded version — and when branding really matters.

A generic medicine contains the same active ingredient, in the same strength and form, as the original branded version. In India, generics are tightly regulated by the CDSCO and must meet identical quality standards.

What's actually the same

  • Active ingredient and strength (e.g. Paracetamol 500 mg)
  • Route of administration (oral, topical, injection)
  • Approved indications and dosing
  • Quality and purity standards

What can differ

  • Inactive ingredients (color, binders, flavour) — rarely matter, but matter to people with allergies
  • Tablet shape, size and packaging
  • Price (often a 5-10× difference)

When branding actually matters

For most common medicines, the generic is a smart choice. For certain narrow-therapeutic drugs (some thyroid, epilepsy, blood-thinning medicines), your doctor may ask you to stick to one brand for consistent dosing.
